Direct Billing Information

We direct bill to most insurance companies, however some may need a physician's note (doctor's prescription) sent to the benefit company directly before we can direct bill.  Some may only cover a percentage of your visit or a maximum amount per visit. Please note some plans do not allow direct billing. Please check with your benefit company before your visit.
